Un nou protocol antibiotic, mai restrictiv, pentru chirurgia de colon este unul dintre principalele rezultate ale proiectului COLONSTRESS, al c\u0103rui obiectiv a fost \u2013 printre altele \u2013 s\u0103 ofere dovezi privind modul \u00een care antibioticele pot altera mediul intestinal, \u0219i nu \u00een mod benefic.<\/p>\n\n\n\n<p>Este bine cunoscut faptul c\u0103 utilizarea ira\u021bional\u0103 a antibioticelor poate afecta grav s\u0103n\u0103tatea. Aceast\u0103 problem\u0103 s-a agravat \u00een timpul pandemiei de COVID-19, c\u00e2nd utilizarea prelungit\u0103 \u0219i excesiv\u0103 a antibioticelor a devenit mai frecvent\u0103 \u2013 adesea f\u0103r\u0103 a fi justificat\u0103 medical. De\u0219i au fost administrate pentru a combate infec\u021biile poten\u021biale, aceste tratamente au avut \u0219i consecin\u021be nedorite: ele nu vizeaz\u0103 doar agen\u021bii patogeni d\u0103un\u0103tori, ci perturb\u0103 \u0219i microbiota benefic\u0103. Dar care este abordarea optim\u0103 \u00een cazul unei interven\u021bii chirurgicale pentru cancer de colon? Una dintre cele mai frecvente complica\u021bii este infec\u021bia postoperatorie. Pentru a o preveni, este necesar\u0103 profilaxia chirurgical\u0103 cu antibiotice \u2013 ceea ce \u00eenseamn\u0103 c\u0103 antibioticul trebuie s\u0103 fie prezent \u00een \u021besut exact \u00een momentul \u00een care rana este deschis\u0103, adic\u0103 atunci c\u00e2nd riscul de infec\u021bie este cel mai ridicat. \u201e<em>Anterior <\/em>\u2013 continu\u0103 Ciobanu \u2013 <em>foloseam antibiotice timp de cinci p\u00e2n\u0103 la \u0219apte zile \u00eenainte de opera\u021bie pentru a preveni infectarea pl\u0103gii<\/em>\u201d. Asta \u00eensemna o \u00eenc\u0103rc\u0103tur\u0103 mare de antibiotice pentru pacient, pe parcursul mai multor zile, cu un stres major asupra microbiomului. Dar dac\u0103 ar exista o alternativ\u0103 mai eficient\u0103 \u0219i mai pu\u021bin invaziv\u0103? De exemplu, administrarea unei singure doze de antibiotic la \u00eenceputul anesteziei. Prin proiectul \u2013 finan\u021bat prin programul ENI CBC Rom\u00e2nia\u2013Republica Moldova \u2013 echipe de cercetare din ambele \u021b\u0103ri au comparat dou\u0103 protocoale antibiotice diferite pe grupuri similare de pacien\u021bi, desf\u0103\u0219ur\u00e2nd teste paralele pentru a identifica cea mai eficient\u0103 abordare. Patru sute de pacien\u021bi au fost testa\u021bi, c\u00e2te 200 \u00een fiecare \u021bar\u0103, iar 70 dintre ei au fost testa\u021bi de dou\u0103 ori, \u00een Chi\u0219in\u0103u \u0219i Ia\u0219i, pentru a valida metoda \u00een ambele institu\u021bii partenere. \u201e<em>Cercetarea noastr\u0103<\/em> \u2013 spune Ciobanu \u2013 <em>a demonstrat c\u0103 administrarea unei singure doze la \u00eenceputul anesteziei este mai eficient\u0103 \u00een prevenirea complica\u021biilor postoperatorii, f\u0103r\u0103 a provoca modific\u0103ri ale microbiomului colonic uman<\/em>\u201d.<\/p>\n\n\n\n<p>Cercetarea a fost o component\u0103 esen\u021bial\u0103 a proiectului. De fapt, unul dintre obiectivele COLONSTRESS a fost instruirea cercet\u0103torilor din Republica Moldova \u00een secven\u021bierea ADN, \u00een special \u00een metagenomica bacterian\u0103. Dup\u0103 cum explic\u0103 cercet\u0103torul Valeriu \u021au\u021buianu: \u201e<em>Deoarece colegii no\u0219tri din Ia\u0219i (Rom\u00e2nia) aveau mult mai mult\u0103 experien\u021b\u0103 \u00een acest domeniu, ne-au ghidat la fiecare pas \u2013 de la izolarea probelor p\u00e2n\u0103 la interpretarea datelor. \u00cempreun\u0103 \u2013 <\/em>continu\u0103 el<em> \u2013 am ajustat protocolul, fiabile pentru utilizare clinic\u0103<\/em>.\u201d Iar p\u00e2n\u0103 la finalul activit\u0103\u021bilor, cele dou\u0103 institute de oncologie partenere au reu\u0219it s\u0103 stabileasc\u0103 o can significantly lower the risk of complications in colorectal surgery.
